ABSTRACT

Objective:To explore the application effect of Internet-based hospital-institution linkage care management model in elderly care for the elderly of dementia.Methods:The research was a quasi-experiment study. A total of 80 elderly people of dementia from three combined medical and nursing institutions, Beijing Fangshan District Mental Health Care Hospital, Beijing Jinhai Hospital of Traditional Chinese Medicine, and Beijing Yiciyuan Nursing Care Center, were selected as the research objects by convenient sampling. The elderly people of dementia enrolled from May to August 2021 served as the control group, and elderly people of dementia enrolled from September to December 2021 served as the experimental group, with 40 cases in each group. Conventional management methods were taken in the control group, while Internet-based hospital-institution linkage care management model was taken in the experimental group. Before the intervention, at 3 months of intervention, the Mini-Mental State Examination (MMSE), Barthel index (BI) and Cohen Mansfield agitation behavior inventory (CMAI) were used to evaluate the cognitive function, activities of daily living and agitation behavior of two groups.Results:A total of 78 cases were included, including 39 cases in the experimental group and 39 cases in the control group, respectively. Before intervention, there was no significant difference in scores of MMSE, BI, and CMAI in the two groups of elderly people with dementia ( P>0.05). After 3 months of intervention, the score of MMSE in the experimental group was (17.36±5.67) points, which was higher than (15.05 ± 2.70) points of the control group, and the difference was statistically significant ( t=2.13, P<0.05). The score of CMAI in the experimental group was (34.18 ± 4.37) points, which was lower than (37.13 ± 5.06) points of the control group, and the difference was statistically significant ( t=2.76, P<0.05). There was no significant difference in BI scores ( t=0.13, P>0.05). Conclusions:The Internet-based hospital-institution linkage care management model improves the cognitive function and agitated behavior of the elderly with dementia in medical-nursing institutions.

ABSTRACT

ObjectiveTo investigate the targets and mechanism of Baofeikang granules in the treatment of pulmonary fibrosis based on network pharmacology and verify the predicted mechanism based on animal experiment. MethodThe active ingredients and targets of Baofeikang granules were screened via the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform, and the targets of pulmonary fibrosis were searched in various disease databases. The common targets shared by Baofeikang granules and the disease were extracted for the establishment of the protein-protein interaction (PPI) network in STRING. Cytoscape 3.8.0 was used to analyze the network topology of the key targets and to establish the ''active ingredient-target'' network. Gene ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analyses were performed on the core targets to explore their possible molecular mechanisms. The rats were assigned into four groups: normal group, model group, prednisone acetate group, and Baofeikang granules group. The rat model of interstitial lung fibrosis was established by tracheal instillation of bleomycin. After 21 days of gavage, the lung tissues of rats were stained with hemotoxylin and eosin (HE) for the observation of morphological changes, and phosphatidylinositol 3-kinase (PI3K) and protein kinase B (Akt) were detected via immunohistochemical (IHC) staining. ResultBased on network pharmacology, 18 key targets of Baofeikang granules were identified for the treatment of pulmonary interstitial fibrosis, including Akt1, mitogen-activated protein kinase (MAPK) 1, myelocytomatosis oncogene (MYC), hypoxia-inducible factor-1α (HIF-1α), cyclin-dependent kinase inhibitor 1A (CDKN1A), epidermal growth factor receptor (EGFR), and Runt-related transcription factor (RUNX2). KEGG pathway enrichment predicted that Baofeikang granules exerted anti-fibrotic effect mainly through PI3K/Akt, tumor necrosis factor (TNF), and interleukin-17 (IL-17) signaling pathways. The IHC results in animal experiment showed that the protein levels of PI3K and Akt were lower in the Baofeikang granules group than in the model group (P<0.05, P<0.01). ConclusionBaofeikang granules has low toxicity, multiple targets, and multiple pathways in the treatment of pulmonary fibrosis. It may alleviate pulmonary fibrosis through regulating PI3K/Akt signaling pathway, so as to improve the lung function.

ABSTRACT

Objective@#To explore the prognosis and risk factors of pyelectasis in high-risk infants.@*Methods@#This was a retrospective study. Totally 960 high-risk infants, who accepted type B ultrasonic examination for fetus at 28th week of gestation and for newborns in 48 hours after birth, were included in the study in departments of obstetrics and eonatology, Shunyi Maternal and Children's Hospital of Beijing Children's Hospital during May 2012 to April 2013. The degree of pyelectasis was classified using Grignon grade and the paients were followed up for 3 years. The factors of epidemiology, high risk pregnant women, fetus and high-risk newborns that relate to pyelectasis were summarized. High-risk factors were analyzed by using logistic multivariate regression analysis.@*Results@#Of 960 high-risk infants, 103 had abnormal urinary ultrasound results, 87 (9.1% of high-risk infants) were diagnosed with pyelectasis, 16 (1.7% of high-risk infants) were diagnosed with congenital anomalies of the kidney and urinary tract. According to the degree of pyelectasis, 68 infants were Grignon grade Ⅰ, male:female ratio=5.8∶1, left side:right side ratio=1.91∶1; 19 infants were Grignon grade Ⅱ, male:female ratio=5.33∶1, left side:right side ratio=2.12∶1. Postnatal follow-up results showed that pyelectasis disappeared in 48 cases (55% of pyelectasis cae), 40 infants were Grignon grade Ⅰ (59% of all Grignon grade Ⅰ patients), 8 infants were Grignon grade Ⅱ (42% of all Grignon grade Ⅱ patients); The result of risk factors analysis showed that the risk of pyelectasis in males was 4.368 times that of females (95%CI: 2.33-8.189, P<0.05); the risk of pyelectasis in low birth weight infants was 22.434 times that of non low birth weight infants (95% CI: 5.883-85.547, P<0.05).@*Conclusion@#The incidence of pyelectasis in high-risk infants was 9.1%. The mitigation rate of pyelectasis in Grignon grade Ⅰ to Ⅱ in fetal or newborn period is high. Patients in Grignon grade Ⅲ and above in fetal or new born period had high risk of congenital anomalies of the kidney and urinary tract. The risk of pyelectasis of male was higher than that of female; the risk of pyelectasis of low birth weight infant was higher than appropriate for gestational age infants.

ABSTRACT

Objective To investigate the current status and existing problems in the implementation of stroke care continuity in China, and to collect relevant suggestions for solving these problems. Methods Focus group interviews were used, to survey thirty-six nursing managers and senior nurses in neurology departments or rehabilitation programs in 24 cities of 13 provinces. The interview team members presided over the meetings, with full-time staff taking notes and recordings, and the results of the interviews were summarized and organized in a timely manner after the interview. Results At present, most of the general hospitals surveyed are doing their best to continue supporting nursing care for patients discharged from hospitals in different forms following their rescue of the acute phase. However, they are faced with such challenges as insufficient nursing manpower, and discontinuity between different medical institutions, which result in oversimplified nursing care and fragmented care. Conclusions Stroke nursing practitioners are working on continuous nursing care, with quite some challenges as well. Nursing care continuity is an important step towards establishing and improving a scientific and rational grading nursing care system.

In this paper, various aspects of the development of pressure sores in recent years were summarized, consisted of the main risk factors, pathogenesis, treatment and care methods. To control pressure sores should focus on prevention, and reflect the integrity and the target population.

ABSTRACT

Objective To examine the effect of labor analgesia with ropivacaine on maternal serum prolactin, time of first eolostrum production and the rate of abundant lactation. Methods A total of 124 women of vaginal delivery were randomly divided into labor analgesia group (n = 75 ) and control group (n =49). Labor analgesia group received ropivacaine by patient-controlled epidural analgesia. Three ml ropivacaine (0.125% ) was injected through an epidural catheter and another 12 ml ropivacaine was injected 5 min later if there were no total spinal anesthesia. The block level of analgesia was controlled to be below T10 level. Then 5 ml (0.104 mg/min) ropivacaine per hour was continuously pumped till full dilation of ostium of the uterus. The control group consisted of women of normal spontaneous delivery with no pain relieving measure. The prolactin levels of antepartum, postpartum 0 h, 2 h, 6 h, 12 h and 24 h were determined by microparticle chemoluminescence. Starting time of lactation, the feeding times in 24 hours,the rate of abundant lactation, and neonatal weight 24 hours after delivery were recorded. Results ( 1 ) The serum prolactin of both groups increased instantly after delivery, reached a peak 2 hours after delivery and kept high levels 24 hours after delivery. (2)The prolactin levels of labor analgesia group [(19. 5±8.4)nmol/L and ( 14. 5 ± 5.6 ) nmol/L] were lower than those of control group [( 22.6±7. 2 ) nmol/L and ( 16. 9 ± 5.7 ) nmol/L] 2 and 24 hours after delivery ( P < 0. 05 ). ( 3 ) In labor analgesia group the starting time of lactation was within 24 hours after delivery in 73 cases (97%), lactation amount was abundant within 48 hours in 55 cases (73%) and newborn weight reduction in the first day after delivery was(57 ±42)g. In control group the starting time of lactation was within 24 hours after delivery in 45 cases (92%),lactation amount was abundant within 48 hours in 28 cases (57%) and newborn weight reduction in the first day after delivery was( 62±40)g. There were no differences between the two groups in the starting time of lactation, the rate of abundant lactation, and newborn weight reduction ( P > 0. 05 ). Conclusions ( 1 )Epidural anesthesia with ropivacaine might affect the secretion of prolactin, while the starting time and amount of lactation may be affected by other factors. (2) Prolactin increases after delivery, reaches a peak 2 hours after delivery and maintains high levels 2.4 hours after delivery, which contented necessary for lactation.
